Reporting Guidelines for CCN Grantees
Periodic reports are necessary not only for proper oversight to ensure accountability, but also serve as a valuable learning tool for grantees and the Communities Connect Network staff. We greatly appreciate your candor and thoughtful review of your organization’s experience.
Current grantees should refer to "Attachment 3" in the sub-grant agreement for a schedule of report due dates. The report should include a narrative of two to four pages and financial attachments. The report should focus specifically on the project supported by your grant. Please submit two complete copies to the grant manager at Communities Connect, details below:

- Describe the progress towards the measurable grant outcomes that were outlined in your proposal. What was the actual impact for participants due to your program? If the project delivered services, indicate numbers of clients served and demographic information such as race/ethnicity, age, gender and geography. Have your numbers increased or decreased since the grant was awarded?
- Describe your successes or accomplishments.
- What challenges did you encounter and how did you respond? Please give specific examples highlighting any changes in the project plan, timeline, staff or finances that affected the project’s outcomes.
- Summarize the methods and results of the program evaluation. Please describe the key lessons learned, and how they have been shared internally and externally. Describe your future goals and plans for the program.
- What are your current funding sources for this program? Were you able to leverage additional funds because of the Communities Connect grant? Has your budget increased or decreased? Please discuss your fundraising plans for ongoing sustainability.

Using the attached spreadsheet report on actual versus budgeted expenditures. Include how Communities Connect funding was expended. Explain any significant variances (10% +/-) in your narrative report. Please include a list of additional funding sources, including amounts received.
